CardiAction is The Care Group's multi-disciplinary approach to the identification, treatment, and management of coronary artery disease and congestive heart failure. It is nurse managed and physician directed. The program's objective is to take "Action" against "Cardiac" disease. CardiAction acts as an "umbrella" over three specific areas: the Cardiovascular Risk Factor Management Clinic (smoking cessation, lipid treatment, cardiac rehabilitation), Endocrinology and the Congestive Heart Failure (CHF) Clinic.
